| South Bay of Los Angeles -- Providence Trinity Care Hospice in Torrance has an immediate opening for a Nurse Practitioner with Hospice/Palliative Medicine training or experience to join our outstanding Hospice program. Position works with all ages, children and adults, in the palliative care/hospice arena. Primarily home-visit care; flexible depending on qualifications and past work experience. Blend of hospice and palliative care cases. Prefer candidates with pediatric, oncology, critical care, hospice/palliative and/or geriatric experience. Providence TrinityCare Hospice is a Medicare-certified hospice recognized in the Southern California region and the nation for its strong commitment to quality end-of-life patient care. About the Providence Facility: Providence TrinityCare Hospice is the oldest and largest not-for-profit hospice program in the Greater Los Angeles area. We have a special way of caring for people with life-limiting illnesses that focuses on the physical, emotional, social and spiritual needs of the whole person. The Hospice has five clinical teams and a staff of more than 180 dedicated professionals who provide compassionate end-of-life care and services to more than 30,000 patients and families in our community. Providence TrinityCare Hospice has received special commendations from the Community Health Accreditation Program (CHAP) in 2002, 2005 and 2008, in recognition of our development and support of innovative programs. Providence TrinityCare Hospice also partners with all five Providence medical centers in the Southern California region to provide palliative and hospice care consultations to patients in hospitals and long-term care settings.
